Palliative Support
La Famia's Palliative support is a service dedicated to clients and their families facing a serious or life-threatening illness.
La Famia family managers work in partnership with families when their loved one is in the terminal phase of an illness.
What is palliative support?
La Famia’s palliative support is a professional service provided to individuals of all ages with a life limiting illness where no cure is apparent and the medical prognosis of recovery is essentially abandoned. Support services extend to our client’s family or whanau, friends and in some instances to the community as needed.
Who is eligible for palliative care?
All people in Canterbury with a life limiting illness should have access to palliative support resources and services that are culturally appropriate and provide an atmosphere that encourages the maximum comfort, dignity and quality of life for the individual client,the family or whanau and the community.
What are the qualifications ofthe people providing care?
All La Famia professional caregivers are pre-qualified as La Famia family managers through our family management course curriculum. This includes a thorough background screening. Members of the palliative support team have extensive experience and training in social and community work; case management, rest home and hospital staff experience and training; as well as personal and family counselling and support.
What can palliative support provide for me and my family?
- Scheduled visits with the client and family pre-discharged from the hospital or in the home.
- Support during needs assessment and care co-ordination with existing private Non-Government-Organisations, as well as hospital care and public agency services.
- Case management liaising with other support services such as regular nursing and medical specialists, as well as the provision of personal care as needed.
- Ongoing psychological counselling and support referrals for the caregivers, clients, and family or whanau, including links to public and private financial and bereavement support services.
Will my doctor continue to care for me?
Your La Famia palliative support professional will work closely with your family doctor as well as co-ordinate and liaise with all prescribed specialized support services as required to ensure that you get the best care possible.
La Famia Palliative Support Services has been established in the Canterbury district to provide a unique family centered approach to people facing a serious, life-threatening or terminal illness.
Our key objectives in this offering are three fold:
- o ensure that all clients diagnosed with a life limiting illness are discharged from hospital with informed knowledge, respect, support and continuity of services.
- To enable clients and their families to make their own decisions and remain self-determining via way of support from La Famia’s palliative care family managers.
- To assist the family and loved ones in rebuilding their lives and reconnecting with their community.
